Job summary

Ascension in Flint, MI is seeking a Social Service professional for a Day shift, full-time position in the PACE Michigan facility. This role focuses on conducting assessments, coordinating treatment plans, and connecting participants with community resources to support post-discharge goals.

Your work will involve interdisciplinary collaboration to address evolving health and social needs, with a focus on participant-centered care and accurate documentation.

Qualifications

• Masters degree in Social Work required.
• 3 years of experience preferred.
• Limited or full licensure in the State of Michigan required.

Responsibilities

• Conduct interviews and assessments to establish participant priorities and ensure accurate case documentation.
• Coordinate treatment plans with interdisciplinary teams, agencies, and caregivers to ensure compliance.
• Facilitate referrals to community resources and support programs for participants and caregivers.
• Build strong community partnerships to address participants' evolving health and social needs.
• Manage concurrent reviews and discharge planning, coordinating social services and post-discharge support.
