Job summary

Crown Hospice in Cape Girardeau, MO is seeking a compassionate Social Worker for a part-time role (20 hours per week) to support patients and families at end of life. The position offers flexibility and meaningful, patient-centered care in a hospice setting.

Ideal candidates hold a BSW/MSW, Missouri license, and at least 1 year of healthcare experience. CPR certification and reliable transportation are required or obtainable.

Qualifications

• Bachelor's or Master's degree in Social Work (BSW or MSW).
• Current Missouri Social Work license (or eligibility).
• Minimum one year of healthcare experience (hospice or home health preferred).
• Excellent communication, organization, and relationship-building skills.
• Current CPR certification (or willingness to obtain).
• Valid driver's license, reliable transportation, and automobile insurance.

Responsibilities

• Provide emotional support and counseling to patients and families navigating end-of-life care.
• Complete psychosocial assessments and develop individualized plans to meet each patient's unique needs.
• Connect patients and families with community resources, advance care planning, financial assistance, and supportive services.
• Serve as an advocate, educator, and trusted guide throughout the hospice journey.
• Collaborate with nurses, physicians, chaplains, hospice aides, and other team members to provide exceptional, patient-centered care.
