About the Role
Legacy Hospice is seeking a skilled and empathetic Social Worker (SW) to provide psychosocial support and care coordination for patients and families facing a terminal diagnosis. In this role you will help patients and families cope with emotional, social, and practical challenges, bringing comfort and clarity during their most vulnerable moments.
What You’ll Do
- Conduct psychosocial assessments of patients and caregivers
- Provide counseling to patients and families regarding emotional stress, coping, grief, and advance care planning
- Assist with advance directives, funeral arrangements, and navigating legal/financial decisions
- Identify and coordinate with community resources, agencies, and support organizations
- Educate staff and families about cultural and psychosocial needs
- Document patient conditions, interventions, and psychosocial support provided
- Serve as a liaison between families and care teams to ensure coordinated, holistic care
- Participate in interdisciplinary team meetings to develop individualized care plans
- Provide crisis intervention and support for safety concerns (neglect, abuse, self‑harm)
- Participate in quality improvement initiatives and continuing education
- Participate in on‑call support as needed
Qualifications
- Bachelor’s degree in Social Work (BSW) required
- Master’s degree (MSW) or LCSW strongly preferred
- At least 1 year of experience in social work, preferably in hospice, home health, or a community setting
- Knowledge of end‑of‑life care, grief support, and cultural sensitivity
- Strong written and verbal communication skills
- Self‑motivated, organized, and able to work independently
- Current Basic Life Support (CPR) certification
- Valid driver’s license, auto insurance, and reliable vehicle required
- Legacy Hospice is a drug‑free workplace. Pre‑employment drug screening is required, including testing for marijuana.
