Job summary

Phoenix Home Care in St. Louis, MO is seeking a Hospice Social Worker to support patients and families at the end of life in home settings. You’ll provide counseling and coordinate care with the hospice team to improve quality of life.

The role requires a MSW or LCSW and strong communication, multitasking, and professionalism. Hospice experience is preferred, with a commitment to compassionate, client-centered care.

Qualifications

• Masters in Social Work or LCSW required.

Responsibilities

• Provide individual and family counseling in the home setting.
• Assist with establishing advance directives and durable power of attorney.

• Develop and facilitate support groups.
• Collaborate with Hospice team and other Social Workers.
• Maintain professional relationships with staff and referral sources.

Hospice Social Workers offer care and assistance to individuals in the final stage of life, as well as their families and close ones. Their purpose is to provide emotional and practical support, with the goal of enhancing the well‑being of those confronting end‑of‑life challenges. Hospice social workers collaborate closely with healthcare teams to organize treatments, services, and available resources, ultimately aiming to enhance a patient's overall quality of life.
