Job summary

Crown Hospice in Cape Girardeau, MO is seeking a Part-Time Hospice Social Worker (BSW/MSW) to provide compassionate psychosocial support. The role offers a flexible 20-hour weekly schedule with competitive pay of $27-$29 per hour, DOE.

You will collaborate with an interdisciplinary team to support patients and families at the end of life. Qualifications include a social work degree, state licensure eligibility, and at least one year of healthcare experience.

Qualifications

• Bachelor's or Master's in Social Work (BSW/MSW) required.
• Current Missouri Social Work license or eligibility for licensure.
• Minimum one year of healthcare experience; hospice/home health preferred.
• Excellent communication, organization, and relationship-building skills.
• CPR certification or willingness to obtain; valid driver's license with reliable transportation.

What You'll Do

As a valued member of our interdisciplinary hospice team, you'll:

• Provide emotional support and counseling to patients and families navigating end-of-life care.
• Complete psychosocial assessments and develop individualized plans to meet each patient's unique needs.
• Connect patients and families with community resources, advance care planning, financial assistance, and supportive services.
• Serve as an advocate, educator, and trusted guide throughout the hospice journey.
• Collaborate with nurses, physicians, chaplains, hospice aides, and other team members to provide exceptional, patient-centered care.

What You'll Bring

Required Qualifications:

• Bachelor's or Master's degree in Social Work (BSW or MSW).
• Current Missouri Social Work license (or eligibility for licensure, as applicable).
• Minimum one (1) year of healthcare experience (hospice or home health preferred).
