Job summary

Phoenix Home Care and Hospice is seeking a dedicated Hospice Social Worker to provide in-home counseling and support for patients at end of life and their families in St. Louis, MO. This role integrates with the healthcare team to coordinate care and resources.

The ideal candidate holds an MSW or LCSW, preferably with hospice experience, and demonstrates strong communication, professionalism, and teamwork in a home-based setting. Monday–Friday, 8am–5pm, with a supportive work environment.

Qualifications

• MSW degree or LCSW licensure.
• Hospice experience preferred.
• Ability to work independently and in a team.

Responsibilities

• Provide in-home individual and family counseling.
• Assist with advanced directives and durable power of attorney.
• Facilitate use of community and home care resources.
• Attend and participate in Interdisciplinary Group (IDG) meetings.
• Refer to long-term care facilities when needed.
• Develop and facilitate support groups.
• Collaborate with hospice staff and referral sources.

Hospice Social Workers offer care and assistance to individuals in the final stage of life, as well as their families and close ones. Their purpose is to provide emotional and practical support, with the goal of enhancing the well-being of those confronting end-of-life challenges. Hospice social workers collaborate closely with healthcare teams to organize treatments, services, and available resources, ultimately aiming to enhance a patient's overall quality of life.

Job Duties

• Provide individual and family counseling in the home setting
• Assist with establishing advanced directive and durable power of attorney
• Facilitate the utilization of community and home care resources
• Attend and participate in IDG
• Make referrals to long-term care facilities as needed
• Develop and facilitate support groups
• Meet with other Social Workers and Hospice team
• Maintain professional relationship with staff and referral sources

Requirements

• Masters-in-Social Work or Licensed Clinical Social Worker (LCSW)
• Hospice experience preferred
• Ability to work in a professional manner
