Job summary

Crown Hospice in Victoria, TX is seeking a Licensed Hospice Social Worker (LBSW, LMSW, or LCSW) to join our compassionate interdisciplinary team. You’ll provide emotional support to patients and families navigating end-of-life care and help develop individualized psychosocial plans.

Required: MSW from an accredited program, Texas MSW license, 1 year healthcare experience (hospice preferred), strong communication, CPR, and a valid driver’s license. This full-time role offers a competitive salary.

What You'll Do

As a valued member of our interdisciplinary hospice team, you'll:

• Provide emotional support and counseling to patients and families navigating end-of-life care
• Complete psychosocial assessments and develop individualized plans to meet each patient's unique needs
• Connect patients and families with community resources, advance care planning, financial resources, and supportive services
• Serve as an advocate, educator, and trusted guide throughout the hospice journey
• Collaborate with nurses, physicians, chaplains, aides, and other team members to provide exceptional, holistic care
