Job summary

Bishop McCarthy Center in New Jersey is seeking a Social Worker (LSW/CSW) to support residents with emotional adjustment, conduct assessments, and participate in care planning.

You will collaborate with interdisciplinary teams, document progress, and assist with discharge planning and education on advanced directives. Join a compassionate, team-oriented facility focused on resident well-being and professional growth.

Qualifications

• Bachelor's degree in Social Work from an accredited College is required.
• Must obtain a CSW and/or LSW licensure.
• One to Three years of professional experience and/or training; or equivalent combination of education and experience.

Responsibilities

• Assists residents with emotional adjustment to facility.
• Interview new residents and collect social history and assessments.
• Completes the social services portion of the MDS, care planning, baseline, etc.
• Refer to psychological services as needed.
• Schedule and participate in interdisciplinary care plan meetings and related activities.
• Writes summaries and progress notes as directed.
• Assist with pre-discharge evaluation and arranging Visiting Nursing Services.
• Assist with discharge planning and follow-up after discharge.
• Conduct Cut Letters and write discharge summaries.
• Develop professional relationships with residents and families.
• Educate residents/representatives on Advanced Directives.
• Adhere to Social Services Regulations.

Responsibilities for a social worker

• Assists residents with emotional adjustment to facility.
