Job summary

Bishop McCarthy Center is seeking a Social Worker, LSW/CSW to join our team in New Jersey. The role focuses on emotional support, social histories, and care planning to help residents adapt to the facility.

Responsibilities include completing MDS social services, coordinating with psychology services, and attending care plan meetings. CSW/LSW certification and licensure are required, with 1–3 years of relevant experience encouraged.

Qualifications

• Bachelor's degree in Social Work from an accredited college is required.
• CSW and/or LSW certification must be obtained.
• 1–3 years of professional experience or relevant training preferred.

Responsibilities

• Assists residents with emotional adjustment to the facility.
• Interview new residents and collect social history and assessments.
• Complete social services portion of MDS, care planning, and baseline data.
• Refer residents to psychological services as needed.
• Attend interdisciplinary care plan meetings and participate in morning meetings.
• Write summaries and progress notes as directed.
• Assist with discharge planning and post-discharge follow-up.
• Develop professional relationships with residents and families.
• Educate on Advanced Directives and adhere to social services regulations.

Responsibilities for a social worker

• Assists residents with emotional adjustment to facility.
• As directed, interview new residents and records, social history, and assessments.
• Completes the social services portion of the MDS, Care planning, baseline, etc.
• Refers to psychological services as needed.
• Schedules and attends and participates in interdisciplinary care plan meetings morning meetings, utilization review, etc.
• Writes summaries and progress notes as directed.
• Assists in conducting a pre-discharge evaluation including but not limited to setting up Visiting Nursing Services, resources, etc. in the community.
• Assists with discharge planning. Follows up after discharge.
• Conducts required Cut Letters
• Writes discharge summaries as assigned.
• Develops a professional relationship with each resident and family.
• Educates residents/ representatives on Advanced Directives.
• Adheres to all Social Services Regulations.

Qualifications for a social worker

• Minimum Bachelor's Degree in Social Work from an accredited College.
• Must obtain a CSW and/or LSW.
• One to Three years of professional experience and/or training; or equivalent combination of education and experience.
