Job summary

Bishop McCarthy Center For Rehabilitation & Healthcare is seeking a Social Worker (LSW/CSW) to support residents and families in a team-oriented facility in New Jersey. The role focuses on emotional adjustment, assessments, care planning, and discharge planning, with opportunities for professional growth and excellent compensation based on experience and licensing.

Qualifications

• Bachelor's degree in social work from an accredited college.
• CSW and/or LSW licensure required.
• One to three years of professional experience and/or training.

Responsibilities

• Assists residents with emotional adjustment to facility.
• Interview new residents and record social history and assessments as directed.
• Completes the social services portion of the MDS, care planning, baseline, etc.
• Refer to psychological services as needed.
• Schedule and participate in interdisciplinary care plan meetings and related tasks.
• Write summaries and progress notes as directed.
• Assist with pre-discharge evaluations and planning, including referrals.
• Assist with discharge planning and follow-ups after discharge.
• Conduct required cut letters and write discharge summaries.
• Develop professional relationships with residents and families.
• Educate residents/ representatives on Advanced Directives.
• Adhere to social services regulations.

Responsibilities for a social worker

• Assists residents with emotional adjustment to facility.
• As directed, interview new residents and records, social history, and assessments.
• Completes the social services portion of the MDS, Care planning, baseline, etc.

• Writes summaries and progress notes as directed.
• Assists in conducting a pre-discharge evaluation including but not limited to setting up Visiting Nursing Services, resources, etc. in the community.
• Assists with discharge planning. Follows up after discharge.
• Conducts required Cut Letters
• Writes discharge summaries as assigned.
• Develops a professional relationship with each resident and family.
• Educates residents/ representatives on Advanced Directives.
• Adheres to all Social Services Regulations.

Qualifications For a Social Worker

• Minimum Bachelor's Degree in Social Work from an accredited College.
• Must obtain a CSW and/or LSW.
• One to Three years of professional experience and/or training; or equivalent combination of education and experience.
