Job summary

Bishop McCarthy Center is seeking a Social Worker LSW/CSW to support residents' emotional well-being and care planning in a team-focused environment.

You will interview new residents, document social histories, assist with MDS processes, and coordinate discharge planning while educating families on advanced directives.

Qualifications

• Bachelor's Degree in Social Work from an accredited College.
• Must obtain a CSW and/or LSW.
• One to Three years of professional experience and/or training; or equivalent combination of education and experience.

Responsibilities

• Assists residents with emotional adjustment to facility.
• Interview new residents and records, social history, and assessments as directed.
• Completes the social services portion of the MDS, care planning, baseline, etc.
• Refers to psychological services as needed.
• Schedules and attends interdisciplinary care plan meetings morning meetings, utilization review, etc.
• Writes summaries and progress notes as directed.
• Assists in discharge planning including coordinating Visiting Nursing Services and community resources.
• Develops professional relationships with residents and families.
• Educates residents/representatives on Advanced Directives.
• Adheres to all Social Services Regulations.

Responsibilities for a social worker

• Assists residents with emotional adjustment to facility.
• As directed, interview new residents and records, social history, and assessments.
• Completes the social services portion of the MDS, Care planning, baseline, etc.
• Refers to psychological services as needed.
• Schedules and attends and participates in interdisciplinary care plan meetings morning meetings, utilization review, etc.
• Writes summaries and progress notes as directed.
• Assists in conducting a pre-discharge evaluation including but not limited to setting up Visiting Nursing Services, resources, etc. in the community.
• Assists with discharge planning. Follows up after discharge.
• Conducts required Cut Letters
• Writes discharge summaries as assigned.
• Develops a professional relationship with each resident and family.
• Educates residents/ representatives on Advanced Directives.
• Adheres to all Social Services Regulations.
