Job summary

The Bishop McCarthy Center For Rehabilitation & Healthcare is hiring a Social Worker (LSW/CSW) to support residents’ emotional well-being and discharge planning. The role emphasizes teamwork within a caring facility to recruit, retain and reward staff.

The position requires a CSW/LSW with 1–3 years of experience and a commitment to social services regulations, patient advocacy, and coordinated care planning. Competitive benefits and overtime opportunities are highlighted.

Qualifications

• Bachelor's degree in Social Work from an accredited college.
• Must obtain CSW and/or LSW.
• One to three years of professional experience and/or training.

Responsibilities

• Assist residents with emotional adjustment to the facility.
• Interview new residents and records, social history, and assessments.
• Complete the social services portion of the MDS, Care planning, baseline, etc.
• Refer to psychological services as needed.
• Schedule and attend interdisciplinary care plan meetings, morning meetings, utilization review, etc.
• Write summaries and progress notes as directed.
• Assist in discharge planning and pre-discharge evaluations.
• Educate residents/ representatives on Advanced Directives.
• Adhere to all Social Services Regulations.

Responsibilities for a social worker

• Assists residents with emotional adjustment to facility.
• As directed, interview new residents and records, social history, and assessments.
• Completes the social services portion of the MDS, Care planning, baseline, etc.
• Refers to psychological services as needed.
• Schedules and attends and participates in interdisciplinary care plan meetings morning meetings, utilization review, etc.
• Writes summaries and progress notes as directed.
• Assists in conducting a pre-discharge evaluation including but not limited to setting up Visiting Nursing Services, resources, etc. in the community.
• Assists with discharge planning. Follows up after discharge.
• Conducts required Cut Letters
• Writes discharge summaries as assigned.
• Develops a professional relationship with each resident and family.
• Educates residents/ representatives on Advanced Directives.
• Adheres to all Social Services Regulations.

Qualifications For a Social Worker

• Minimum Bachelor's Degree in Social Work from an accredited College.
• Must obtain a CSW and/or LSW.
• One to Three years of professional experience and/or training; or equivalent combination of education and experience.
