Summary of Job Duties: The Social Work Program Manager will provide psychoeducation for patients and their support systems, and disease self-management counseling for those living with dementia. The Social Work Program Manager will complete consultations with individuals, couples, and families to include functional and strength-based assessment after diagnosis; administer and interpret brief objective symptom measures, as requested by the Director; and coordinate with Infusion Center program staff for follow-up after treatment. The Social Work Program Manager will provide evidence-based/evidence-informed brief interventions to individuals, families, and community partners, including a focus on adjustment to diagnosis, grief counseling, crisis intervention, coping with relationship and/or behavioral changes. Additional focus to include safety and ability to provide adequate level of care at home. The Social Work Program Manager develops and manages patient and family programming focusing on support and education; consults with program team members to enhance continuity of care regarding patient/family needs; develops/facilitates evidence-based treatment or psychoeducational groups as appropriate to patient/family needs.
Qualifications:
  • Master’s degree in Social Work or related field plus five (5) years of program management or related clinical experience required.

  • Current Social Worker license to practice in Arkansas.

Knowledge, Skills & Abilities:

  • Excellent interpersonal skills.

  • Ability to work collaboratively with staff, patients, families, and other community partners.

  • Demonstrates independence and self-motivation, requiring little to no supervision or oversight;


Additional Information:

Responsibilities;

  • Works with the Program Director and other members of the team to plan individual, family, and community caregiver education and support programs and trainings.

  • Develops and maintains relationships with local partners (physicians, clinic, facilities, in home care providers, hospital, Alzheimer’s Arkansas and Alzheimer’s Association, AR Chapter)to promote patient outcomes as well as professional standards.

  • Conducts individualized assessments, which will lead to evaluating the patient and family caregivers' needs ·

  • Develops and implements Individualized support and education for community caregivers.

  • Uses a variety of interactive educational methods to enhance the learning experience.

  • Presents continuing education programs as needed.

  • Implements policies and procedures identified for the training programs.

  • Participates in the promotion of the programs and provides consultation on policy recommendations for patients with dementia and caregivers within UAMS and the state of Arkansas to the Director of the Memory Clinic and the Chair of the Department of Geriatrics.

  • Oversees all facets of the program, on-site and remotely·

  • Serves as a resource to other staff members who provide education, consultation, referral, and resource services to patients, family members, and /or caregivers

  • Performs other duties as assigned.
