ChristianaCare is hiring a full-time Social Worker II for the Primary Care at Home program serving New Castle County, DE. You will provide compassionate counseling, care planning, and ongoing support to patients in their homes, addressing social determinants of health and enabling coordinated, patient-centered care.

Requires an MSW with 1–2 years of related experience preferred; DE LMSW/LCSW preferred. Travel to homes in New Castle, DE is a key aspect of the role, with hours Mon-Fri

Qualifications

• Master’s degree in social work from an accredited graduate program.
• Delaware LMSW or LCSW preferred.
• 1–2 years of related work experience preferred.

Responsibilities

• Provide social work and discharge planning services for patients with complex needs.
• Identify social, emotional, and environmental needs and connect to resources.
• Support care transitions and coordinate with interdisciplinary team.

Education
