Job summary

WeCare Centers atAvalon Springs Care Center in Mercer, PA, seeks a Social Work Director to lead resident services and coordinate interdisciplinary care.

You will oversee care plans, MDS, and quality improvement while ensuring health and safety, reporting to administration, and advocating for a high-quality living environment for residents and their families.

Qualifications

• Bachelor's degree in Social Work or Human Services required.
• Previous experience in geriatrics or long-term care preferred.

Responsibilities

• Reviews and follows up on any concerns for the residents and staff with all incident reports.
• Works with caregivers of the nursing unit and all departments to provide the residents with a fulfilling living experience.
• Keeps supervisor informed of unusual occurrences that relate to the health and safety of residents.

• Keeps Admin fully informed of stats, progress and achievements.
• Must have the ability to deal tactfully with personnel, residents, family members, visitors and the general public.
• Must be able and willing to work well with inter-disciplinary teams.
