WeCare Centers is seeking a Social Work Director for our Mercer, PA community. The role involves supervising social work services, coordinating with care teams, and ensuring high-quality resident support.

Candidates should hold a Bachelor's in Social Work or related Human Services field and have experience in geriatrics or long-term care. Interdisciplinary collaboration and leadership are essential to succeed in this position.

Qualifications

• Bachelor's Degree in Social work or Human Services.
• Experience in geriatrics or long-term care.
• Ability to work with interdisciplinary teams.

Responsabilités

• Reviews and follows up on concerns for the residents and staff with all incident reports.
• Works with caregivers of the nursing unit and all departments to provide the residents with a fulfilling living experience.
• Keeps supervisor informed of unusual occurrences that relate to the health and safety of residents.
• Oversees care plans, MDS, and quality improvement processes.
• Keeps Admin fully informed of stats, progress and achievements.
• Must have the ability to deal tactfully with personnel, residents, family members, visitors and the general public.
• Must be able and willing to work well with inter-disciplinary teams.
• Any other duties as assigned.
