Qualifications

Responsibilities

• Identify patients who may benefit from case management services.
• Complete psychosocial assessments to evaluate needs.
• Develop, implement, and monitor psychosocial treatment plans.
• Provide crisis intervention, counseling, and family support.
• Coordinate referrals to community resources and services.
• Advocate for patients to address unmet needs and barriers to care.
• Collaborate with physicians, nurses, and the care team on discharge planning.
• Document assessments and care plans in the EMR per standards.
