Qualifications

• Basic computer skills.
• Successful completion of Acts 73 and/or 169 clearances within 90 days of commencing employment, if applicable.

Responsibilities

• Perform complex transition of care post hospitalization.
• Conduct psychosocial assessment.
• Provide consultation, education, community linkage, crisis intervention, and support services for referred patients and families.
• Support the provision of health care services and appropriate utilization of resources across the continuum of care.
