Job summary

Heritage Valley Sewickley is seeking a Case Management social worker to lead complex transitions of care after hospitalization, including psychosocial assessments and community linkage.

The role collaborates with care teams to support crisis intervention and family navigation of resources. A Bachelor’s degree in social work or related field and one year of supervised health-care experience are required, with weekend/holiday rotation and on-call as needed.

Qualifications

• Bachelor's degree in social work or related field.
• One year supervised social work experience in a health care setting.
• Basic computer skills.
• Successful completion of Acts 73 and/or 169 clearances within 90 days of commencing employment, if applicable.

Responsibilities

• Responsible for complex transition of care post hospitalization, including psychosocial assessment, consultation, education, community linkage, crisis intervention, and support services for referred patients and families.
