Responsibilities

• Contributes to and completes initial and ongoing comprehensive assessment and coordinates care across the continuum.
• Collaborates to provide safe transitions for discharge and connects post-discharge care with community resources.
• Educates patients and families about post-discharge care and access to community support; communicates with the care team and payers.
• Provides crisis interventions for abuse, mental health, and substance use issues; supports goals of care.
