Corewell Health's Royal Oak facility is seeking a part-time social worker to collaborate with an interdisciplinary team to assist patients and families with complex psychosocial needs. Qualifications

• Master's Degree in Social Work is required.
• Experience in individual/family assessment, crisis intervention, grief counseling, and discharge planning preferred.
• MI MSW licensure required upon hire; state-specific MSW credentials encouraged.

Responsibilities

• Conduct psychosocial assessments and develop care plans for identified needs.
• Communicate with alternate care sites and community agencies to address patient needs.
• Document assessments and plans in EMR, reflecting current condition and transition needs.
• Provide consultation and resources to the healthcare team.
• Maintain knowledge of community resources.
• Participate in department or hospital committees as assigned.
• Develop knowledge of federal/state regulations related to the role.
• Inpatient duties: daily group therapy and discharge planning approvals as needed.
