Job summary

WellSpan Health is seeking a social worker to plan, organize, and implement psycho-social support for patients and families. The role works in a Care Continuation Team model and coordinates discharge planning with physician practices to address transition needs.

Responsibilities include psychosocial assessments, referrals to counseling and community resources, and collaboration with the interdisciplinary team to ensure safe, effective care transitions. Weekend scheduling is available.

Qualifications

• Bachelor's degree in social work or social welfare required.
• Less than 1 year relevant experience required.
• BLS required upon hire; LCSW or LSW preferred upon hire.

Responsibilities

• Assess psycho-social needs of patients and families during hospitalization.
• Identify discharge plans and coordinate cross-continuum care.
• Document assessments, plans, barriers, and progress in medical records.
• Interdisciplinary collaboration to develop discharge plans and education.
