Job summary

Mercy Social Worker II is responsible for facilitating discharge planning from admission to discharge in collaboration with the healthcare team. The role supports patients with complex psychosocial needs and coordinates with a multidisciplinary staff to ensure safe transitions.

The position requires a Master’s in Social Work (MSW) or pending MSW, and a current LMSW/LCSW license in the state of employment, with 0–2 years in an acute care hospital setting.

Qualifications

• Master's in Social Work (MSW) or awaiting conferment for MSW.
• Current license in Social Work in the state of employment (LMSW, LCSW).
• 0–2 years experience in acute care hospital setting.
• BLS (CPR) at hire date, or within 90 days.

Responsibilities

• Issues discharge planning from admission to discharge with the care team.
• Intervene with patients with complex or high-risk psychosocial needs.
