Mercy STL is seeking a Social Worker II to provide psychosocial assessments, counseling, case management, patient education, and discharge planning as part of an interdisciplinary team. This full-time, day-shift role focuses on patients and families, advocating for care coordination and addressing behavioral and social factors impacting recovery.

Qualifications include an MSW and LMSW/LCSW licensure, with 0–2 years of related experience; hospital or behavioral health background preferred.

Qualifications

• Master's Degree in Social Work (MSW) required.
• Current LMSW or LCSW license in Missouri required.
• 0–2 years social work experience required; hospital or behavioral health experience preferred.

Responsabilités

• Perform psychosocial assessments and develop care and discharge plans.
• Provide counseling, crisis intervention, education, and case management.
• Coordinate care with physicians, nursing staff and community providers.
• Assist patients/families in accessing community resources and follow-up services.
• Advocate for patients with behavioral health and substance use concerns.
• Identify abuse, neglect, and safety concerns and comply with reporting requirements.
• Maintain documentation and participate in treatment planning.
