Job summary

Mercy provides social work services for patients and families to enhance psychosocial functioning in an acute care hospital setting. This role supports discharge planning, care coordination with the interdisciplinary team, and community resource linkage.

As a Social Worker II, you will perform assessments, advocate for patients, and ensure adherence to Mercy standards while maintaining confidentiality and quality care.

Qualifications

• MSW required or awaiting conferment.
• Current LMSW/LCSW licensure in the state of employment.
• 0–2 years in acute care hospital setting.

Responsibilities

• Provide therapy, counseling, case management, education and discharge planning for behavioral health patients.
• Complete initial psychosocial assessments and document in medical records.
• Advocate for patients with the Interdisciplinary Team and coordinate discharge planning.
