AdventHealth Minneola is seeking an experienced Care Manager to provide grief counseling, disease adjustment support, crisis intervention, and goals of care planning for patients. You will assess discharge needs across inpatient, observation, and ER settings to prevent delays and coordinate timely post-acute care.

You will review medical records, develop discharge plans with contingency options, collaborate with post-acute services, and ensure complete medical records and smooth transitions.

Competenze

• Master's degree required in social work or related field.
• Experience in care management and social work preferred.
• Strong communication and assessment skills.
