AdventHealth Winter Garden is seeking a dedicated care coordinator with a focus on grief counseling, discharge planning, and post-acute care coordination. The role requires collaboration across inpatient, observation, and emergency departments to ensure timely discharge and safe transitions.

Master's degree and 2+ years in care management or social work are preferred, with state guidelines knowledge and strong communication skills. This on-site position offers day shifts and full-time hours.

Responsabilités

• Provides grief counseling, disease adjustment support, crisis intervention, goals of care planning, and de-escalation as needed.
• Assesses discharge planning needs for inpatients, observation, and ED patients to avoid delays.
• Reviews medical records and integrates clinical, social, and financial factors into care plans.
• Develops discharge plans with contingency plans for post-acute care and community services.
• Uses technology and standard work to communicate with post-acute providers and ensure continuity of care.
• Performs other duties as assigned.
