AdventHealth in Palm Coast, FL, seeks a skilled Care Manager to support discharge planning and post-acute care coordination across inpatient, ED and observation units. You will assess patient needs, mobilize community resources, and collaborate with a multidisciplinary team to ensure timely, patient-centered transitions.

Requirements include a Master's degree and 2+ years in care management or social work; licensure such as ACM/CCM preferred.

Formación

• Master's degree required.
• Care management experience preferred.
• Social work background required.

Responsabilidades

• Provides grief counseling, disease adjustment support, crisis intervention, goals of care planning support, and de-escalation services for patients as appropriate.
• Assesses patients’ and families’ wholistically for discharge planning needs in inpatient settings.
• Reviews medical records and incorporates clinical, social, and financial factors into discharge plans.
• Develops discharge plans and arranges post-acute care services and community care.
• Communicates with post-acute services to ensure continuity of care and accurate medical records.
• Participates in multi-disciplinary rounds to review status and plan resources for discharge.
