The role involves conducting psychosocial assessments, coordinating admissions and transfers, and advocating for residents' rights while maintaining dignity and privacy. We value teamwork, communication, and compassion in daily interactions with residents, families, and staff.

Qualifications

• Enjoys interacting with the elderly and strong communication skills.
• Problem-solving abilities and basic computer skills.
• Experience in a Long Term Care Facility.
• Good interpersonal skills.

Responsibilities

• Interview Residents to develop social history and psychosocial assessments.
• Orient Residents and families to the facility and Residents' Rights.
• Assist with intake, admission, and transfers.
• Coordinate inspection of belongings for labeling and inventory.
• Address resident concerns promptly using the concern form.
• Provide 1-to-1 visits with Residents as requested.
• Assist Residents to participate in activity programs.
• Report hazardous conditions and equipment or supply issues.
• Answer call lights promptly and courteously.
• Maintain privacy, comfort and dignity with Residents.
• Communicate effectively with Residents, visitors, families, peers, supervisors.
• Assist with answering phones and paging when needed.
