Yanceyville Rehabilitation And Healthcare Center in Yanceyville, NC is seeking a Social Work Assistant to join our long-term care team. This 40-hour-per-week position supports residents' emotional and social needs in coordination with care plans.

Qualifications

• Enjoys interacting with the elderly; strong communication and problem‑solving skills.
• Proficient with computers and basic office software.
• Experience in a Long Term Care Facility preferred.

Responsibilities

• Interview residents to develop social history and psychosocial assessments.
• Orient residents and families to the facility and residents' rights.
• Coordinate intake, admission and transfers within the facility.
• Assist with labeling and inventory of residents' belongings.
• Provide 1-to-1 visits with residents as requested.
• Assist residents with activity programs of choice.
• Report hazards, equipment and supply issues; follow safety precautions.
• Respond to calls promptly and courteously in care areas.
• Maintain privacy and dignity of residents and interact with warmth and respect.
• Communicate effectively with residents, visitors, families, peers and supervisors.
