Yanceyville Rehabilitation And Healthcare Center in Yanceyville, NC is hiring a Social Work Assistant to support residents' emotional and social needs within a long-term care setting. You will work with residents, families, and the care team to coordinate assessments, care plans, and referrals, while upholding privacy and dignity.

This role emphasizes compassionate communication, coordination, and timely response to concerns, with 40 hours per week and opportunities for advancement.

Qualifikationen

• Excellent communication and problem-solving abilities.
• Ability to interact warmly with elderly residents and families.
• Capable of coordinating assessments and care plans.
• Comfortable using basic computer systems and records

Aufgaben

• Interviews Residents to develop social histories and psychosocial assessments.
• Orient Residents and families to the facility and Residents' Rights.
• Assist with intake, admission, and transfers within the facility.
• Coordinate inspection of Residents' belongings for labeling and inventory.
• Assist Residents with concerns, complete concern forms, and respond promptly.
• Provide 1-to-1 visits with Residents as requested.
• Assist Residents in participating in Activity Programs of choice.
• Report hazards, equipment issues, and supply needs; follow safety procedures.
• Respond to call lights promptly and courteously in care areas.
• Maintain privacy, comfort, and dignity of Residents; interact with warmth and respect.
• Communicate effectively with Residents, Visitors, families, peers, and supervisors.
• Assist with answering phones and transferring calls when needed.
