Yanceyville Rehab and Healthcare in Yanceyville, NC is seeking a Director of Social Services to oversee the Social Services Department and ensure the psychosocial well‑being of Residents. This leadership role requires coordination of care plans, regulatory compliance, and collaboration with families and staff.

The position involves supervising staff, developing discharge plans, connecting Residents with community resources, and maintaining a respectful, dignified environment.

Qualifications

• Bachelor’s degree in social work or similar field.
• A minimum of 1 year experience in Long Term Care/Nursing Facility.

Responsibilities

• Responsible for keeping up-to-date evaluation documentation on each Resident’s activities in compliance with regulations.
• Attends care conferences and identifies psychosocial needs for care plan inclusion.
• Coordinates intake, admission, and transfers within the facility.
• Oversees psychosocial group assessments and documentation.
• Mediates issues among Residents, families, and staff.
• Provides linkage with community resources and referrals as needed.
• Develops discharge plans with Residents and families in collaboration with staff and agencies.
• Provides counseling to support Residents’ socialization and activities.
