Job summary

Yanceyville Rehab and Healthcare in Yanceyville, NC, seeks a Director of Social Services to manage day-to-day operations of Social Services, oversee coordination and evaluation of care plans, and ensure the psychosocial well-being of Residents.

The role requires a bachelor's in social work (or equivalent) and at least two years of healthcare social work experience, with strong counseling and leadership abilities. Budget management and staff supervision are included.

Qualifications

• Bachelor's degree in social work or similar field.
• Two years' experience in a social worker role, preferably in a health care setting.
• Strong communication and counseling abilities.

Responsibilities

• Maintain up-to-date evaluation documentation for residents in compliance with Federal, State, and Local regulations.
• Attend care conferences and identify psychosocial needs for inclusion in the care plan.
• Coordinate intake, admission, and Resident transfer within the facility.
• Coordinate inspection of Resident belongings to ensure labeling and inventory.
• Attend Interdisciplinary Team and Unit Care conferences; supervise psychosocial groups.
• Mediates issues among Residents, families, and staff.
• Provide linkage with community resources by maintaining knowledge and making referrals.
• Develop a comprehensive discharge plan with Residents and families.
• Provide informal counseling to address issues impacting socialization.
• Maintain filing system for the Social Services Department.
• Operate the department within budget and conduct annual performance reviews.
• Train and educate staff; provide counseling and disciplinary action as needed.
• Report hazards and ensure safety precautions.
• Maintain Residents' comfort, privacy and dignity; communicate respectfully.
• Respond promptly to call lights; participate in meetings and trainings.

Summary/Objective

In keeping with our organization's goal of improving the lives of the Residents we serve, the Director of Social Services manages the day-to-day operations of the Social Services Department and responsible for the overall administration, coordination, and evaluation of the social services function to meet and maintain the mental and psychosocial well-being for each Resident.

Essential Functions

1. Responsible for keeping up-to-date evaluation documentation on each Resident's activities at the facility which complies with Federal, State, and Local regulations.
2. Attends scheduled care conferences and identifies and interprets psychosocial needs of Residents for inclusion in the care plan, addressing in particular issues of schedule, treatment and environment that express and reinforce individuality and identity.
3. Assists with the coordination, intake, admission, and Resident transfer within the facility.
4. Coordinates the inspection of Resident's belongings to ensure they are properly labeled and inventoried.
5. Attends Interdisciplinary Team and Unit Care conferences, Assesses Residents for psychosocial groups, and supervises implementation and documentation.
6. Mediates issues that arise among Residents, families, and staff.
7. Provides linkage with appropriate community resources by maintaining knowledge of other systems, making referrals, and identifying unmet needs (e.g., recreational transportation, adaptive phone equipment, etc.)
8. Develops a comprehensive discharge plan in collaboration with Residents, Residents' families, multidisciplinary staff, and referral agencies.
9. Provides informal counseling when needed to uncover any problems which might interfere with the Residents' socialization and participation in activities.
10. Establishes, develops, maintains and updates filing system for the Social Services Department.
11. Responsible for operating the Social Services Department within budgetary guidelines and limitations.
12. Completes annual performance reviews of all subordinate staff. Provides guidance and education to staff in regards to their performance.
13. Provides counseling and disciplinary action to subordinate staff members as needed.
14. Responsible for training and educating staff members.
15. Reports all hazardous conditions, damaged equipment and supply issues to appropriate persons.
16. Follows established safety precautions when preforming tasks and using equipment and supplies.
17. Maintains the comfort, privacy and dignity of Residents and interacts with them in a manner that displays warmth, respect and promotes a caring environment.
18. Communicates and interacts effectively and tactfully with Residents, visitors, families, peers and supervisors.
19. Answers and respond to call lights promptly and courteously when working in Resident care areas.
20. Attend and participate in departmental meetings and in-services as directed.

Supervisory Responsibility

This position supervises the Social Worker and other subordinate staff.
