Sunrise Nursing & Memory Care seeks a compassionate Social Work Assistant to support residents' psychosocial well-being in a skilled nursing setting. The role offers a flexible 3-day-per-week schedule, with days chosen to fit personal needs and facility needs.

You will assess resident needs, coordinate services, and support care planning while ensuring compliance with CMS regulations, MDS, and NASW standards.

Competenze

• Bachelor's degree in Social Work, Human Services, Psychology, or related field.
• 1+ year of social work experience in healthcare or long-term care.
• Knowledge of MDS, care planning, and CMS regulations strongly preferred.
• Strong communication, organization, and documentation skills.

Mansioni

• Conduct psychosocial assessments and complete social histories.
• Participate in MDS assessments, care plans, and interdisciplinary meetings.
• Provide emotional support and adjustment counseling for residents and families.
• Coordinate discharge planning and community resource referrals.

Position Summary

We are seeking a compassionate and detail-oriented Social Work Assistant / Social Services Designee to support the psychosocial well-being of residents in our skilled nursing facility.

This role offers a flexible 3-day-per-week schedule, allowing you to select the days that work best for you.

You will assess resident needs, coordinate services, and support care planning while ensuring compliance with CMS regulations, MDS, and NASW standards.

Key Responsibilities

• Conduct psychosocial assessments and complete social histories
• Participate in MDS assessments, care plans, and interdisciplinary meetings

Qualifications

• Skilled nursing facility (SNF/LTC) experience preferred
• Knowledge of MDS, care planning, and CMS regulations strongly preferred
• Strong communication, organization, and documentation skills
