Job summary

Sunrise Nursing & Memory Care seeks a compassionate Social Work Assistant / Social Services Designee to support residents’ psychosocial well-being in a skilled nursing facility. You will assess needs, coordinate services, and support care planning in line with CMS regulations, MDS, and NASW standards. Familiarity with SNF processes is preferred.

Qualifications

• Licensed or certified in Social Work or Human Services.
• Bachelor's degree in Social Work, Human Services, Psychology, or related field.
• 1+ year of social work experience in healthcare or long-term care.
• Knowledge of MDS, care planning, and CMS regulations preferred.

Responsibilities

• Conduct psychosocial assessments and complete social histories.
• Participate in MDS assessments, care plans, and interdisciplinary meetings.
• Provide emotional support and adjustment counseling for residents and families.
• Coordinate discharge planning and community resource referrals.
• Maintain documentation (MDS, care plans, progress notes, PASARR).
• Support resident rights, advocacy, and quality of life initiatives.
• Assist with survey readiness and CMS/state compliance.
• Investigate concerns and support abuse prevention and reporting protocols.
• Collaborate with nursing, therapy, and administration teams.

Position Summary
