Job summary

WeCare Avalon Springs Care Center is seeking a Social Work Assistant in Mercer, PA to support residents and collaborate with a multidisciplinary team. The role focuses on coordinating care plans, addressing concerns, and ensuring a high quality of life for residents.

You will work with caregivers across departments, monitor health and safety, and contribute to quality improvement efforts while fostering compassionate, resident-centered care.

Qualifications

• Bachelor’s degree in Social Work or Human Services required.
• Experience in geriatrics or long-term care preferred.
• Ability to work with interdisciplinary teams in a care setting.

Responsibilities

• Review and follow up on resident and staff concerns with incident reports.
• Collaborate with caregivers and all departments to enhance residents’ living experience.
• Inform supervisor of unusual health or safety occurrences.
• Oversee care plans, MDS, and quality improvement processes.
• Keep Admin informed of statistics, progress and achievements.
• Work tactfully with personnel, residents, families, visitors and the public.
• Collaborate with interdisciplinary teams on care delivery.
• Perform other duties as assigned.

Key Responsibilities

• Reviews and follows up on any concerns for the residents and staff with all incident reports.
• Works with caregivers of the nursing unit and all departments to provide the residents with a fulfilling living experience.
• Keeps supervisor informed of unusual occurrences that relate to the health and safety of residents.
• Oversees care plans, MDS, and quality improvement processes.
• Keeps Admin fully informed of stats, progress and achievements.
• Must have the ability to deal tactfully with personnel, residents, family members, visitors and the general public.
• Must be able and willing to work well with inter-disciplinary teams.
• Any other duties as assigned

Skills, Knowledge and Expertise

• Bachelor’s Degree in Social work or Human Services.
• Previous experience in geriatrics or long term care.
