Job summary

University of Illinois Chicago is seeking a dedicated professional to join the Division of Specialized Care for Children (DSCC) in Champaign, IL. The hybrid role supports care coordination for children with special health care needs, assists with eligibility determinations, records maintenance, and outreach activities, including translation when needed.

Under supervision, the candidate will engage with families, providers, and community resources, participate in trainings, and contribute to DSCC

Qualifications

• Two years of work experience comparable to a Social Work Aide II.
• Ability to assist in determining eligibility for services and coordinating care.
• Experience in public/customer service settings.

Responsibilities

• Assist Care Coordinators and families with care coordination activities and determine DSCC eligibility for applicants.
• Record and transmit demographic and medical- social data; complete forms and letters in support of care coordination services.
• Assist in obtaining private/public health insurance and support DSCC outreach programs and events.
