Job summary

Genesis HealthCare in Hurricane, WV seeks a Social Services Specialist to work with patients and families to identify social, emotional, and mental health needs and coordinate services to enhance quality of life. Qualifications

• Bachelor's degree in social work or human services field is required.
• Must possess any certifications/licensures as required by State of employment to practice in long-term care.

Responsibilities

• Collaborate with the interdisciplinary team to promote and protect patient/resident rights and well-being.
• Participate in the development of a written, interdisciplinary plan of care for each patient/resident to address social service needs.
• Hold meetings with patients/residents, families, and significant others as requested and to provide support and information to improve quality of life.
• Perform patient/resident discharge planning activities such as family/patient education, nursing center documentation and make referrals to appropriate agencies and suppliers.
• Establish relationships and maintain contact and referral flow with community-based agencies/services that benefit our patient/resident population.
