Job summary

• High school diploma or GED required.
• Bachelor’s degree from an accredited college or university preferred.

Responsibilities

• Complete a minimum of 25 household SNAP, Medicaid, CHIP, and TANF applications per month.
• Conduct virtual and in-person outreach for parents and staff across CPS schools.
• Collaborate with schools to ensure Medicaid Designee presence.
• Build partnerships with community organizations and state agencies.
• Develop and operationalize processes to support enrollment, engagement, and utilization of care improvements.
