Job summary

Las Mercedes Management is seeking a Social Services Coordinator in Hollywood, FL to assist patients with access to social, financial, and community resources. You will help connect patients to Medicaid, SNAP, QMB, SSI, and other services, acting as a liaison among patients, healthcare providers, and agencies.

The role requires evaluating eligibility, completing and submitting applications, and maintaining confidentiality.

Qualifications

• High school diploma or equivalent required.
• 1–2 years of experience in social services, healthcare access, or community resources coordination.
• Knowledge of Medicaid, SNAP, SSI, and related programs strongly preferred.
• Bilingual (English/Spanish or English/Creole) highly preferred.

Responsibilities

• Conduct screenings to identify patients’ eligibility for benefits (Medicaid, QMB, SNAP, LIS, SSI, etc.).
• Assist with completion and submission of applications, renewals, and required documentation.
• Maintain patient confidentiality and ensure compliance with HIPAA and organizational policies.
• Follow up with patients to confirm application status and address pending requirements.
• Provide information about local, state, and federal resources, including housing, transportation, food programs, and financial assistance.
• Refer patients to external agencies or community partners as needed.
• Build and maintain relationships with government agencies and community resource providers.
• Maintain accurate case notes, logs, and reports in designated systems.
• Track applications and outcomes to ensure follow-up and accountability.
• Assist with departmental reporting and data collection.
