Humanitary Medical Center Tampa, Inc. is hiring a Social Services Coordinator to guide patients on social rights, benefits and local resources in a fast-paced healthcare setting.

The role emphasizes HIPAA compliance and coordinating socio-economic support across care teams. You will assess needs, arrange home visits and connect enrollees with community services while liaising with health plans, providers, and families.

Qualifications

• Experience in an acute care, managed care, or social services environment.
• Minimum of High School diploma or equivalent.
• Proficient in Microsoft Office and EHR software.
• Excellent customer relations experience.
• Proficient in Microsoft Office, including Outlook and Excel.

Responsibilities

• Provide information and guidance to patients about their social rights, benefits and existing resources.
• Coordinate home assessments with members and families.
• Assist members in understanding implications of medical conditions and care.
• Develop plan of care to obtain authorizations for home and community-based services.
• Liaise between the Health Plan, providers, enrollees, and families.
• Follow up with members telephonically and/or in-person.
• Promote confidentiality and HIPAA compliance.
