As a Social Services Director / PRSD at Oak Park Oasis, you will play a pivotal role in delivering exceptional care to our residents, ensuring seamless transitions, and promoting independence. About the Role

As a Social Services Director / PRSD, you will be responsible for:

• Developing and implementing individualized care plans that address the social, emotional, and psychological needs of our residents

• Conducting comprehensive assessments to identify and address any barriers to discharge and develop strategies to overcome them

• Collaborating with our interdisciplinary team to ensure continuity of care and effective communication with residents, families, and healthcare providers

• Coordinating outside services and resources to support the transition of residents back to their homes

• Monitoring and evaluating the effectiveness of care plans and making adjustments as needed

• Master's Degree required

Responsibilities
