Job summary

Oak Park Oasis in Oak Park, Illinois, seeks a Social Services Director/PRSD to lead social services, coordinate transitions, and promote resident independence. A Master’s degree is required to oversee discharge planning, policy development, and team supervision within a Joint Commission Approved facility.

You will collaborate with the medical director, families, and providers to ensure seamless care and effective resource utilization, driving quality improvements and resident outcomes.

Responsibilities

• Develop and implement policies and procedures for social services, ensuring compliance with regulations and accreditation standards.
• Supervise and mentor social work staff, providing guidance and support to ensure high-quality services.
• Develop and maintain relationships with community resources, healthcare providers, and other stakeholders to ensure effective referral and discharge planning.
• Participate in quality improvement initiatives to reduce re-hospitalization rates and improve patient outcomes.
• Collaborate with our medical director and other healthcare professionals to ensure comprehensive care.
