Job summary

Life Care Centers of America in Michigan seeks an experienced Social Services Director to lead the department and ensure the emotional and social needs of residents are met in accordance with laws and Life Care standards.

The role requires an active LMSW in Michigan, prior director/manager experience in long-term care, and the ability to coordinate education, patient rights, and family liaison while managing staff.

Qualifications

• Active LMSW license in Michigan is required.
• Two years’ experience in health care social work preferred.
• Management/Director experience in long-term care is preferred.

Responsibilities

• Implement a social services program meeting patient medical, social, and emotional needs within laws and Life Care standards.
• Act as patient advocate and educate staff on patient rights.
• Recruit, hire, train, and supervise social services staff; collaborate on discharge planning.
• Maintain confidentiality and adhere to policies; participate in interdisciplinary teamwork.
