You will oversee resident care management, advocate for resident rights, and ensure social services meet clinical needs with a focus on gerontology.

Qualifications

• Must have current Limited License in Social Work (REQUIRED); LMSW preferred.
• Bachelor’s Degree in Social Work (REQUIRED).
• 3+ years of related experience in a health care facility; long term care experience highly desired.

Responsibilities

• Oversee and direct resident care management and ensure all social services provisions are carried out.
• Identify medically-related social services needs of residents and ensure that these needs are met.
• Assist in meeting the psychological needs of each resident and manage services to improve social functioning and preserve dignity.
• Coordinate psychiatric services and monitor psychoactive medication usage.
