Job summary

Life Care Centers of America in Indiana seeks a Social Services Director to lead the department, ensuring the medical and emotional needs of patients are met in accordance with laws, regulations, and Life Care standards.

You will supervise and develop social services staff, advocate for patients, educate staff on patient rights, coordinate with families and community agencies, and uphold confidentiality and professional conduct.

Qualifications

• Must be licensed in the applicable state and maintain active status.
• Bachelor’s degree required for facilities with 120+ beds.
• Two years’ healthcare social work experience preferred.

Responsibilities

• Implement a social services program meeting the medically related social and emotional needs per guidelines.
• Act as patient advocate and educate staff regarding patient rights.
• Chart appropriately and timely.
• Assist patients and families with education, financial planning, liaison with community agencies, etc.
• Recruit, hire, train, evaluate, counsel, and supervise social services staff.
• Exhibit excellent customer service and a positive attitude towards patients.
• Maintain confidentiality and professional relationships with all associates and vendors.
• Participate in evacuation procedures and adhere to attendance requirements.
• Communicate effectively on an interdisciplinary team.
