Life Care Centers of America in Michigan seeks an experienced Social Services Director to lead the department and ensure the emotional and social needs of residents are met in accordance with laws and Life Care standards.

The role requires an active LMSW in Michigan, prior director/manager experience in long-term care, and the ability to coordinate education, patient rights, and family liaison while managing staff.

Formación

• Active LMSW license in Michigan is required.
• Two years’ experience in health care social work preferred.
• Management/Director experience in long-term care is preferred.

Responsabilidades

• Recruit, hire, train, and supervise social services staff; collaborate on discharge planning.

Seeking an experienced Social Services Director.Well qualified candidates will:

1. Have an active LMSW in Michigan

2. Have experience as a Director or Manager in Social Services in a Long-term Care setting for 1+ years

Other Requirements;

• Approximately (1) Late Day per month
• Manager On Duty Weekend Rotation as assigned

Position Summary

The Social Services Director plans, organizes, develops, and directs the overall operation of the Social Services department to ensure all medically related emotional and social needs of patients are met in accordance with all applicable laws, regulations, and Life Care standards.

Education, Experience, and Licensure Requirements

• Currently registered/licensed in applicable State. Must maintain an active license in good standing throughout employment.
• Bachelor’s degree if working in a facility with 120 or more beds (see State law)
• Two (2) years’ experience in health care social work preferred

Specific Job Requirements

• Make independent decisions when circumstances warrant such action
• Knowledgeable of practices and procedures as well as the laws, regulations, and guidelines governing functions in the post acute care facility
• Implement and interpret the programs, goals, objectives, policies, and procedures of the department
• Perform proficiently in all competency areas including but not limited to: patient rights, and safety and sanitation
• Maintains professional working relationships with all associates, vendors, etc.
• Maintains confidentiality of all proprietary and/or confidential information
• Understand and follow company policies including harassment and compliance procedures
• Displays integrity and professionalism by adhering to Life Care’sCode of Conductand completes mandatoryCode of Conductand other appropriate compliance training

Essential Functions

• Implement a social services program that meets the medically related social and emotional needs of patients as well as State, Federal, corporate, and division guidelines
• Act as patient advocate and provide education to staff regarding patient rights
• Chart appropriately and timely
• Assist patient and family through education, financial planning assistance, liaison with community agencies, etc.
• Recruit, select, hire, train, evaluate, counsel, and supervise social services staff
• Exhibit excellent customer service and a positive attitude towards patients
• Assist in the evacuation of patients
• Concentrate and use reasoning skills and good judgment
• Communicate and function productively on an interdisciplinary team
• Sit, stand, bend, lift, push, pull, stoop, walk, reach, and move intermittently during working hours
• Read, write, speak, and understand the English language
