Job summary

The Blossoms is seeking a social worker to support residents by addressing social, emotional, and psychosocial needs within a long-term/sub-acute care setting. You will help develop individualized care plans and participate in admissions, discharges, and interdisciplinary team meetings.

Requirements include 1+ year in Social Work and at least a related degree (BSW/MSW preferred). Experience in long-term care is a plus; duties include coordinating with finance and ensuring proper documentation.

Qualifications

• At least one year of experience in Social Work field.
• Experience in long term care and/or sub-acute setting preferred.
• BSW/MSW preferred or related Bachelor's degree.

Responsibilities

• Maintaining and/or improving resident’s ability to control everyday physical needs and mental/psychosocial needs
• Identifying individual social and emotional needs
• Developing and maintaining individualized care plans
• Participating in the planning of the resident’s admission, community re-entry or transfer to another facility
• Participating in interdisciplinary care plan meetings to ensure treatment of the social and emotional needs of the resident
• Work with the finance team to ensure resident is educated and the proper documents are in place for the resident during their stay
• Work with the resident on discharge and get necessary DME or home care in place so that the resident is safe when they return to the community
