Job summary

Visalia Post Acute in Visalia, CA is seeking a compassionate Social Services Director for a full-time role. You will assess residents' needs, develop care plans, coordinate discharge planning, and advocate for resident rights within an interdisciplinary team.

The ideal candidate holds a bachelor’s degree in social work (MSW preferred) and has healthcare social-work experience, with strong counseling and communication skills. This on-site position offers competitive pay and comprehensive benefits.

Qualifications

• Bachelor's degree in Social Work, Human Services, Psychology, Sociology, or a related field is required.
• At least two years of supervised social-work experience in a healthcare setting is required.
• Master of Social Work degree is preferred.
• Skilled nursing, long-term care, rehabilitation, or geriatric experience is strongly preferred.
• Knowledge of resident rights, discharge planning, care planning, community resources, and psychosocial assessment.
• Experience with PointClickCare or other electronic health-record systems is preferred.
• Strong counseling, crisis-management, advocacy, and problem-solving abilities.
• Excellent written and verbal communication skills; ability to handle confidential information.

Responsibilities

• Assess residents' social, emotional, psychological, and family needs.
• Develop goals and action plans in collaboration with residents and families.
• Provide medically related social services to support wellbeing.
• Assist with developing, reviewing, and updating social-services care plans.
• Advocate for resident rights, dignity, and quality of life.
• Coordinate discharge planning with residents, families, departments, and external agencies.
• Participate in interdisciplinary care conferences and communicate needs to the care team.
• Provide staff education and in-service training as requested.
