Job summary

Worland Health and Rehabilitation is recruiting a Social Services Director to maintain a robust social services program addressing residents’ physical, mental, and psychosocial needs. You will work with residents, families, and staff to ensure comprehensive planning and resource coordination.

The role emphasizes discharge planning, coordination with interdisciplinary teams, and facilitating access to community resources to support residents’ well-being.

Qualifications

• Strong communication with residents, families, and staff.
• Preferred: a minimum of Bachelor's degree in accredited social work program.

Responsibilities

• Collaborate with residents, families, and community resources to complete social history and discharge planning.
• Develop interdisciplinary care plans addressing bio-psychosocial needs.
• Support residents and parties in locating financial, legal, mental health, and other resources.
• Coordinate social work services to promote psychosocial well-being and appropriate treatment.
• Assist staff with behavioral interventions and discharge follow-up in coordination with the team.

Job Summary

Worland Health and Rehabilitationis currently seeking aSocial Services Directorto assist with maintaining thesocial service program designed to meet the individual physical, mental, and psychosocial needs of the residents.

Essential Functions

• Works with residents, families, and community support to completesocial service history, discharge plan, and development of interdisciplinary care plans designed to meet the bio-psychosocial needs of resident.
• Assisting residents and responsible parties in location and using financial, legal, mental health, and other community resources.
• Coordinate and/or provide health and mental health social work services to residents to assist with attaining or maintaining the highest practical mental and psychosocial well-being, while helping resident who display mental or psychosocial difficulty receive appropriate treatment and services.
• Provide resource support to staff participating in behavioral interventions for residents.
• Coordinating safe discharge planning and follow–up services in coordination with interdisciplinary team.

Education, Licensure, And Experience

• Effective communication skills with residents, families, and staff.
• Preferred: a minimum ofBachelors' Degree in accredited program of social work
